The 35-year-old mother of two young children now heads the business which employs more than 19,000 engineers responsible for the "final mile" of copper cable or fibre optic connecting homes and businesses to BT exchanges.
Garfield, who has long been tipped as one of the top women to watch in UK business, has been promoted from director of strategy and regulatory affairs at BT. She takes over from Steve Robertson who led Openreach since it was hived off from BT in 2005.
She said: "Openreach is a terrific business - underpinning much of the UK economy - so being selected to lead it is a fantastic opportunity and one I am thrilled to have been offered. Fibre broadband is the future and I am determined to make it available to as many homes and businesses as possible via a range of providers."
A modern language graduate from Cambridge, Garfield began her career with consultants Accenture at the age of 21 before joining BT six years later in 2004.
Openreach had revenues of just over £5billion last year and runs one of the biggest vehicle fleets in the UK with more than 20,000.
