The free service is being run with the help of Chelsea and Westminster hospital and will offer men tests for high blood pressure, diabetes and obesity.
The clinic, which will run for four hours before kick-off, will also offer advice on cancer screening, help with stopping smoking and cutting down on alcohol.
Test results can be given on the day of the checks or texted when ready.
Dr Rachael Jones, a consultant at the hospital, said men are more at risk of heart disease and other health problems because they were less likely to go for checks. She said: "In the early stages, many conditions have no symptoms so we would urge fans to come and get checked out at Stamford Bridge."
Former Chelsea player and coach John Hollins, who had life-saving treatment at Chelsea and Westminster, launched the project with his son, BBC presenter and Strictly winner Chris. He said: "It is my local hospital and it will always have a special place in my affections."
