Carl Shepherd, 20, also suffered deep cuts and lost four teeth in the attack outside Wimbledon station shortly after Barcelona's victory on Wednesday night.
He said the drunk Chelsea fans set upon him and his friends as he made his way home to Wandsworth. He was knocked unconscious.
His friends and the four attackers ran away and Mr Shepherd said he woke up outside the station covered in blood. He said he called an ambulance but the crew were unable to find him. He said he was told to wait at nearby Wimbledon police station, from where officers called a second ambulance.
Mr Shepherd said the attack was unprovoked. He said: "They were drunk and angry that Chelsea lost and had to take it out on someone. They probably thought I was a fan of an opposing team. It was really scary." Mr Shepherd, who works as a fundraiser, had dreamed of working as a model and was to have his first professional photoshoot tomorrow.
His mother, Roselyn Shepherd, 47, a travel consultant, said she feared her son's modelling career would be over because of his injuries. She said: "The injuries have made my handsome boy look hideous. It is horrifying football thugs could have destroyed his dreams."
A London Ambulance Service spokesman said: "Although there was difficulty determining where we could find the patient, an ambulance crew arrived at 12.02am, within 15 minutes of the original call."
