Lucas Herrington wandered off from Sharp Lane Primary School in Leeds.
He went missing from the school's nursery for 30 minutes before he was eventually found by a canteen worker.
A spokesman for Sharp Lane Primary School said: "We take the safety of our children extremely seriously, and are known locally for the high level of care we offer.
"We are all deeply distressed that this has happened, and are grateful that Lucas was found by a member of our staff and safely returned to school.
"We have extremely strict procedures for the handover of our children, and immediately began a thorough investigation as to how Lucas could have left the premises unaccompanied.
"We are now working with all our staff and those parents and carers who collect our children to ensure that this unique and unfortunate set of circumstances is never repeated."
The boy's mother Jacqueline said her son managed to cross a busy main road before he was found and returned to school.

"Anything could have happened to him. He could have been picked up off the streets by a complete stranger."
