Barrymore, 55, was taken in for questioning with two other men by detectives in Essex. Sources said the other two arrested men were Justin Merritt and Jonathan Kenny.
Essex police did not name any of the men but said said three suspects - aged 32, 37 and 55 - were being held on suspicion of the murder of Stuart Lubbock.
They were also being questioned about allegations of sexual assault, the force said.
Butcher Mr Lubbock, of Harlow, Essex, was found in a swimming pool at Mr Barrymore's home in Roydon, Essex, in March 2001.
Mr Lubbock died after going to a late night party with Mr Barrymore and some friends.
Post-mortem results showed he had suffered severe internal injuries which suggested sexual assault.
Alcohol plus Ecstasy and cocaine were found in his bloodstream.
A lengthy investigation was carried out by Essex Police but no one was charged with any offence in relation to Mr Lubbock's death.
Mr Barrymore accepted a police caution for drugs offences following the original inquiry. The force reopened the inquiry in December last year after a review of the first investigation.
