Leigh Baker, 48, has been held since Monday over the deaths of four women and a man at Parkfields Residential Home in Butleigh, Somerset.
His wife Rachel, 45, who is facing the same allegations, was released on bail on Wednesday night from Yeovil police station pending further inquires.
Mr Baker can be held by police for questioning until 5.30pm on Thursday, when he could be charged or re-bailed.
The couple, who managed the home for nine years, were initially interviewed following the death of 97-year-old Lucy Cox on New Year's Day.
During the investigation, detectives also decided to exhume the remains of three other former residents in order to carry out toxicology tests and establish whether they had been poisoned.
Answering bail on Monday morning, the Bakers were arrested on suspicion of murdering Mrs Cox, along with Nellie Pickford, 89, Marion Alder, 79, and Fred Green, 81. A fifth "suspicious" death remains unidentified by police.
Mr and Mrs Baker live in Glastonbury with their five-year-old daughter.
Mrs Baker was originally arrested on suspicion of administering a noxious substance on January 5 - four days after the death of Mrs Cox. Mr Baker was arrested two days later.
The couple were bailed and waited more than 10 months as police awaited results of toxicology tests, having taken the dramatic step of ordering exhumations for the first time in Avon and Somerset Police history.
