In interviews with The Mail on Sunday and The Sunday Telegraph, Dr Khan revealed that Diana's death continued to haunt him, making him want to scream at times.
He also disclosed that he had been approached to give evidence at the inquest into the Princess's death, but is unlikely to do so unless he is obliged to.
Dr Khan, who has previously never spoken publicly about his relationship with the Princess, said he was breaking his silence because of speculation that he would attend the inquest to give evidence.
He told the Mail on Sunday that events 10 years ago still dominated his thoughts.
"Sometimes I feel like screaming. There have been very bad times. I have moved on, but it keeps coming back."
The 48-year-old heart specialist recently separated from his 29-year-old wife, but declines to say why the arranged marriage broke down.
He similarly refuses to talk about the circumstances surrounding his split with Diana, whom he first met in 1995.
But he does pay tribute to her and talks of the esteem in which he holds the late Princess, criticising her memorial fountain in the process.
"Creating a fountain is not how you should remember a great person. You put great people up as high as possible. Look at Nelson," he told the Sunday Telegraph.
