Andrew Andrews, 61, was crushed by a JCB digger at a garden centre in Lanarkshire.
Emergency crews got him into an ambulance, but the vehicle became stuck in mud at the scene.
According to reports, the driver tried to free the spinning wheels but the clutch burned out.
Another ambulance arrived within 13 minutes and took him to nearby Wishaw General Hospital, but he was pronounced dead on arrival.
A spokesman for the Scottish Ambulance Service said: "When we arrived at the scene, the conditions were very muddy.
"The ambulance got bogged down and in the process of trying to get free, the gearbox failed. We immediately dispatched another vehicle which arrived within 13 minutes and took him to Wishaw General Hospital."
The tragedy happened at the rear of Sandyholm Garden Centre off Lanark Road, near Lanark.
One of Mr Andrews' sons told the Daily Record: "We don't know what happened exactly and will probably find out more tomorrow."
Police are investigating, and a report is being prepared for the procurator fiscal. Health and safety officials have also been informed.
