A 37-year-old man being cared for in a Glasgow hospital has been described as a "highly probable" case.
The patient was admitted to intensive care last week with a chest infection and tests suggest he may have the virus.
Scottish Health Secretary Nicola Sturgeon said it was not yet known whether flu was a contributing factor to his deterioration or simply coincidental.
The total number of cases confirmed in the UK currently stands at 184.
Meanwhile, more than 40 Birmingham school children recently confirmed as having swine flu were said to be responding well to treatment on Tuesday night.
The 37-year-old, admitted to Glasgow's Victoria Infirmary last Thursday, is not known to have had any contact with any swine flu cases or travelled recently.
Staff and patients who have been in contact with him will be given the appropriate advice and treatment, Ms Sturgeon said.
The man is being treated in isolation and no other patients in the hospital were showing symptoms.
So far 170 cases of the H1N1 virus have been confirmed in England, with 13 in Scotland and one in Northern Ireland.
