A Ford Fiesta, a Vauxhall Astra and a Chrysler Neon were involved in the crash at around 8pm on Saturday on the A595 in Moota, near Cockermouth, Cumbria.
The driver of the Fiesta and two rear seat passengers died, along with the Astra driver.
A woman who was a front seat passenger in the Fiesta has been taken to West Cumberland Hospital, in Whitehaven, after suffering "serious injuries", Cumbria Police said.
The male driver of the Neon and his male front seat passenger suffered minor injuries and have been taken to the same hospital.
The A595 between Bothel and Papcastle was closed for several hours but reopened at 3:30am.
Moota, a barren area of the Lake District, is dissected by a long, straight stretch of the A595, one of the main routes between Carlisle and the south and west of the county.
The cause of the accident is not yet known but an investigation is under way.
