The crash happened in front of crowds of spectators at Dover Harbour.
Kent Police said an investigation into the incident was under way.
A Kent Police spokeswoman said: "At 12.40pm Kent Police was notified of a collision between two powerboats taking part in an organised race during Dover Regatta.
"Four people were travelling in the boats. One of the four, a man in his 40s, suffered fatal injuries and was pronounced dead at the scene."
A South East Coast Ambulance Service spokesman said: "I can confirm one patient has died at the scene due to serious multiple injuries."
Simon Smith, manager of Dover Fire Station, said the collision occurred "well out into the harbour" but that hundreds of spectators were watching it.
He told the BBC: "There were several hundred people watching the event and they carrying on watching the emergency services working there together.
"We had to shield the casualty from the eyes of the people watching. A lot of people were under the impression that it was a display they were watching."
Dover Harbour Board said an investigation into the incident had commenced.
