Maternity ward deaths investigated

The North West London Hospitals NHS Trust is reviewing the three deaths, which occurred between June 2007 and March 2008 at the maternity unit at Northwick Park Hospital.
The unit was put in special measures in 2005 after it was revealed that the death rate for new mothers was far higher than the national average.
Ten women died there between April 2002 and April 2005.
A subsequent Healthcare Commission report found a number of failures contributed to nine of the deaths, including the lack of response to changes in patients' condition and poor staff communication.
The unit was lifted out of special measures in September 2006.
The trust has now begun investigating the latest three deaths and is also carrying out a "broader internal review" of its maternity services".
It confirmed the latest death was on March 20.
The review is due to be completed at the end of May.
According to the trust, a recent assessment of its local female population rated 50% as high risk. It said the trust caters for women with more complex needs and also women who present later in their pregnancy.
