Two others arrested near the scene of the 19-year-old's death in a street in east London remain in custody.
The victim was killed after an altercation with a group of men in the early hours of Saturday in High Road, Ilford.
A second teenager, aged 17, also suffered stab wounds in the incident. He was taken to an east London hospital, where he remains in a serious but stable condition. His injuries are not thought to be life-threatening.
Two men, aged 23 and 24, were arrested soon after the incident and were taken to separate east London police stations.
The Metropolitan Police said a 21-year-old man was arrested in Ilford on Sunday afternoon and taken to an east London police station for questioning.
A post-mortem examination carried out at East Ham Mortuary gave cause of death as stab wounds to the heart.
Police said next of kin have been informed but they were awaiting formal identification before releasing the dead teenager's name.
The 19-year-old is the 28th teenager to meet a violent death in the Greater London area this year.
The stabbing comes more than a month after the last murder of a teenager in the capital - that of Craig Marshall, who was killed outside a block of flats in Acton, west London, on September 29.
