Elizabeth Beever, originally from the Oxford area, was on holiday when she died in the collision between a passenger train and a cargo train, local police said.
A police spokesman said she was travelling with a 48-year-old Canadian man, who suffered minor injuries. He could not confirm what the relationship between the pair was.
The Foreign Office said Ms Beever's next of kin have been informed, but could not confirm where she lived.
The accident happened between Bortigali and Birori, in the province of Nuoro.
The driver of the passenger train was also killed, along with a 52-year-old Italian commuter.
Local police official Fabrizio Mustaro said the trains collided on a single-track section of the line.
One of the trains should have stopped at a nearby station and it was unclear why it did not.
Another local police spokesman said it was thought that seven people had been injured.
The cause of the accident, which happened at around 7.30am local time, is under investigation, but police are working on the theory that there was a "human error".
