Adams, who was a co-host on Channel 5's show Cosmetic Surgery Live with presenter Vanessa Feltz, told celebrity Web site TMZ that he performed a tummy tuck and breast reduction on Donda West before she died Saturday night at age 58.
He said she might have died from a heart attack, pulmonary embolism, or massive vomiting.
But it emerged soon after, that earlier this year, David Thornton, the state medical board's executive director, filed a complaint with the state against Adams that included the arrests.
In a 2006 case, Adams was found guilty of driving with a blood-alcohol level of .08 or greater, and he pleaded no contest in 2003 to driving under the influence, according to the medical records.
They also indicate that in 2001, two malpractice lawsuits against Adams ended in payouts of $217,337 and $250,000.

Her publicist Patricia Green said she passed away "as the result of complications from a cosmetic surgical procedure", but gave no more details.
Last night, the coroner announced the autopsy in Dr. West's death, which was brought forward, had failed to pinpoint the cause of death. But other toxicology tests were still pending.
West was pronounced dead at 8:49 p.m. November 10 after being found unresponsive in her Los Angeles home.
Winter said the death had been reported as one resulting from possible complications from cosmetic surgery performed on November 9. He said: "West was released to her home following the surgery on Nov. 9. On Nov. 10, she was found unresponsive after being in respiratory distress.
"She was taken to Centinela Freeman Regional Medical Centre where efforts to preserve her life proved unsuccessful."
Winter said he would have to wait for toxicology tests to be certain over claims the Dr. West's death was caused by a heart attack or blood clot.
Winter added: "West had several minor medical issues, but I cannot comment on any of those."
It has been reported that West underwent a combined procedure that included a breast reduction and a tummy tuck, and she was operated on by television regular Dr. Adam.
Plastic surgeon, Dr. Andre Aboolian of Beverly Hills, said that West had approached him months ago about having cosmetic surgery.
Aboolian told West two weeks ago that she needed to get a medical clearance before he would agree to do the surgery due to a pre-existing condition, said his publicist, Jo-Ann Geffen. But he never heard back from her.
In a statement issued late on Monday, Kanye West thanked "everyone for the outpouring of support and kind words that have come in from across the country".

West's spokesman said the family "asks for privacy during this time of grief".
The 30-year-old often spoke publicly of his close relationship with his mother. She raised him alone after her husband left when West was three.
The rapper had just landed in London when he heard the news of his mother's death and immediately flew to Los Angeles.
Dr West was an English professor at Chicago State University until 2004, before leaving to work with her son, becoming his manager and CEO of Super Good, the parent company of Kanye West Enterprises.
She was the inspiration for the song, "Hey Mama", on West's 2005 album "Late Registration" in which he sings: "Hey Mama, I wanna scream so loud for you, cuz I'm so proud of you ... I appreciate what you allowed for me. I just want you to be proud of me."
In May, she published the book "Raising Kanye: Life Lessons from the Mother of a Hip-Hop Star" in which she paid homage to her famous son.
