Rommedahl has signed for Dutch giants Ajax for £680,000, while Boro have negotiated a price of £2.5million for 28-year-old Young.
The 28-year-old defender has yet to have a medical at the Riverside and must still agree personal terms but he looks set to follow the Denmark winger out of The Valley after more than 200 matches.
Young, who can play at centre-half or left-back, joined the Addicks for £4million in 2000, but has been given permission by manager Alan Pardew to pursue his hopes of a return to the top flight following Charlton's relegation.
Pardew tried to buy Young when he was in charge of West Ham but has already bought the man he believes can be the former club captain's long-term successor - Yassin Moutaouakil.
Middlesbrough boss Gareth Southgate, meanwhile, is hoping to add the defender to his squad after focussing on his forward line for much of the summer. The Teessiders have already signed strikers Tuncay Sanli and Jeremie Aliadiere since the end of last season.
Rommedahl, meanwhile, is returning to the Eredivisie - where he is a four-time title winner - after three years in London.
The 28-year-old cost £2million when he swapped PSV Eindhoven for Charlton three seasons ago but never quite lived up to his billing, despite going on to make 84 appearances for the club. He is looking forward to a new challenge, but insisted his time in England had not been wasted.
He said: "Ajax fits me perfectly. I hope that Ajax think the same. I wanted to return to Holland very much. I played a lot of years in the Dutch Eredivisie, and I love the positive football that is played there. That absolutely counts for Ajax.
"They play on the attack and always play for prizes. But since I went to England I became more of an all-round player."
