Azam had only just returned from a nine-week eye-gouging ban when he clashed with Borthwick in Gloucester's Guinness Premiership defeat to Saracens on September 27.
The Frenchman will not be able to play again until December 29, which rules him out of Gloucester's first four Heineken Cup matches and six Premiership games.
Gloucester's next match after December 29 is the home league clash against Worcester on January 2.
Earlier on Tuesday night, Ireland prop John Hayes has received a six-week ban for stamping on Leinster forward Cian Healy.
Munster tighthead Hayes was sent off following the second-half incident during last Saturday's Magners League game in Dublin, which Leinster won 30-0. His boot made contact with Healy's face.
Hayes' suspension means he will miss the opening two games of Munster's Heineken Cup campaign, starting against Northampton next Saturday and followed by a home appointment with Treviso.
He must also sit out Magners League games against Edinburgh and Ulster, but Hayes will be available for all three of Ireland's autumn Tests, starting against Australia on November 15 and concluding against world champions South Africa 13 days later.
A three-man Irish Rugby Football Union disciplinary panel, sitting in Belfast, decided his offence warranted a "high end entry level" in terms of sanction. Hayes has the right to appeal his ban.
