Robert Koren's 87th-minute strike had denied the League One outfit victory in normal time after Joel Porter had fired them into a 61st-minute lead.
But Foley and Barker scored within three minutes of each other at the end of the first period of extra-time to make the score 3-1, with the Baggies hitting the crossbar on three occasions.
It was Pools who took the lead after midfielder Gary Liddle was rewarded for his perseverance.
The former Middlesbrough player caught defender Leon Barnett in possession inside his own penalty area and then laid the ball perfectly into the path of the on-rushing Porter, who made no mistake to send the home crowd into raptures.
Just when it looked as though Pools had seen off their more illustrious opponents, Koren curled home a superb right-foot shot with just three minutes remaining to send the tie into extra-time.
The tempo simply increased in the first half of extra-time, with Albion hitting the crossbar three times through Luke Moore, substitute Craig Beattie and skipper Jonathan Greening.
But remarkably Hartlepool finished the opening 15 minutes 3-1 ahead after substitutes Foley and Barker struck with 102 and 105 minutes gone respectively.
