The 36-year-old Welshman tops the list of the sporting honours and there are also gongs for two former England rugby stalwarts Lawrence Dallaglio and Bill Beaumont, plus two record-breaking footballers - Northern Ireland's David Healy, and Arsenal and England women's star Kelly Smith.
Other well-known figures honoured include former ECB chairman David Morgan, president-elect of the International Cricket Council, ex-England footballer Cyrille Regis and motor racing legend John Surtees.
Calzaghe, who was voted BBC's sports personality of the year in December won his 45th consecutive bout against USA's Bernard Hopkins in April to become recognised as the world number one in two divisions. He previously received the MBE in 2003.
Calzaghe said: "For the first time in my life I was knocked out. It's unbelievable when I think back to when I was a kid that one day I would have achieved so much through boxing, and my teachers laughed at me when I said I was going to be a boxer."
Wasps captain Dallaglio, who receives an OBE for services to rugby and charity, retired from professional rugby union last month after guiding his club to victory in the Guinness Premiership final at Twickenham.
He was also previously given an MBE after being part of England's 2003 World Cup-winning team.
Beaumont, who receives a CBE for services to rugby and charity, was also a former England captain and is vice-president of the International Rugby Board.
Previously awarded an OBE, his CBE reflects his work as honorary president of the rugby charity Wooden Spoon aimed at improving the lives of disadvantaged children.
