Davies calmly steered the ball down the centre of the goal and past Forest keeper Paul Smith from 15-yards to give the Glovers an emphatic 5-2 victory on the night and a 5-4 aggregate win.
An unfortunate own goal by Alan Wright and a strike from Marcus Stewart in the final eight minutes saw Yeovil level the tie 3-3 on aggregate with a 3-1 win after 90 minutes to force extra-time.
David Prutton was sent off for a second yellow card in the second minute of injury time before Lee Morris put the Glovers ahead in the tie for the first time just two minutes into extra time only for Grant Holt to respond immediately before Davies sent Yeovil to Wembley.
Earlier Davies' superb solo goal midway through the first half gave Yeovil the lead on the night, which they held until the break.
Scott Dobie equalised with a header from Lewis McGugan's corner early in the second-half to restore Forest's two-goal aggregate lead before the comeback started.
