The 24-year-old Yorkshire seamer, wicketless on his Test debut at Lord's and during the first innings of this match, claimed two in successive overs on the final morning as the tourists collapsed towards another heavy defeat.
They lost five wickets in 10 overs following a second interruption for rain and slumped to 167 for eight at lunch, still 92 runs away from avoiding an innings defeat after resuming 144 runs adrift on 115 for three.
England had begun the day with high hopes of sealing the victory which would complete a whitewash on their final day of Test cricket before the start of the Ashes series in July.
They took the field without two members of their starting line-up, with Matt Prior again sitting out play having suffered a badly bruised right ring finger yesterday, which forced Paul Collingwood to again take over the wicketkeeping duties, while Ravi Bopara also failed to field after suffering a bruised right knee.
But England's biggest frustration at the start of the day was the weather with two interruptions for rain inside the first hour's play, which threatened to help West Indies salvage a draw.
It was only after the players took the field for the third time this morning that England made the breakthrough, with Lancashire seamer James Anderson tempting Lendl Simmons into a loose drive to substitute fielder Scott Borthwick at point.
Just seven balls later, Bresnan claimed his first international victim when new batsman Brendan Nash clipped a short ball off his hips straight to Borthwick at square leg.
He followed that by removing the normally stubborn Denesh Ramdin in his next over - his second wicket in three balls - when he edged to Anderson at third slip to inch England closer to victory.
Anderson strengthened England's position by claiming two wickets before the interval, mesmerising Jerome Taylor with his ability to swing the ball before knocking back his off-stump, and then bowling Sulieman Benn with the final delivery before lunch.
