The Lancashire all-rounder came through a stringent test on the right knee problem which forced him out of Monday's emphatic win at Edgbaston.
Flintoff has replaced Chris Tremlett as England's only change, with captain Paul Collingwood shrugging off the stomach cramps which had prevented him from practice on Wednesday.
Hampshire all-rounder Dimitri Mascarenhas also passed a fitness test on his injured right hand, which also forced him to miss Edgbaston, but failed to win his place back in the line-up.
India also made one change to their line-up, with all-rounder Ajit Agarkar replacing seamer Munaf Patel.
Agarkar, though, had to wait for his chance to impress after captain Rahul Dravid won the toss and decided to bat first.
