The 24-year-old joined Werder in 2005 from Danish outfit Aarhus GF but spent January to June last year on loan at FSV Mainz, scoring four goals in his first seven games, and earned his first international cap in March against Spain.
Andreasen said: "I am pleased that the two clubs have been able to reach an agreement to enable me to sign for Fulham. I enjoyed my time at Werder Bremen and am now excited by a new challenge that awaits me."
He added: "I look forward to playing well for Fulham and competing in the Premier League."
Cottagers manager Roy Hodgson added: "He has all the attributes to succeed at the highest level and has the desire and application to perform well in every game."
Werder's general manager Klaus Allofs told the club's official website, www.werder.de: "Leon thought the possibilities of becoming a first-team player at Werder were not very positive.
"He had already spoken with us several times about a transfer and we have granted him his wish. We have come to an agreement with Fulham over the transfer fee."
Werder coach Thomas Schaaf added: "Leon was always somewhat impatient and wanted things to advance.
"Now he wants to seek his chance somewhere else. We wish him a lot of success in the Premier League."
Another possible new arrival is former Liverpool striker Jari Litmanen who is training with the club.
