The Warwickshire player, who was named as the Bears' vice-captain earlier this year, went to America for a third operation on his troublesome right hip and the prognosis does not look good.
Giles was not named in the England and Wales Cricket Board's 25-man performance squad for the summer and is expected to spend around eight weeks on crutches, while his first-class return will take even longer.
Giles did not feature last season due to a similar complaint but was restored to fitness in time to play in the first two Tests of the ill-fated Ashes tour before losing his place to Monty Panesar.
He told the Daily Telegraph: "The operation was more straightforward than the one I had last summer but I will be on crutches for longer. At this stage I can't say how long I will be out of action, but it could be for most of the summer."
The 34-year-old will see a specialist in June to assess his condition.
