The London club have confirmed that a deal has now been agreed subject to a medical.
Fulham had three bids for Greening rejected during the summer but the player, who had handed in a transfer request, was left out of West Brom's most recent match on Tuesday.
The 30-year-old former Albion captain cited a desire to play in the Premier League as his reason for wanting to leave.
Greening joined West Brom in a £1.25million move from Middlesbrough five years ago and was skipper as the team won promotion in 2007-08.
He began his career at York before being picked up by Manchester United and then sold to Middlesbrough for £2million in 2001 after failing to break into the first-team picture at Old Trafford.
West Brom remain reluctant to part company with the player but admit they have been unable to force him to stay.
Chairman Jeremy Peace told the club's website, www.wba.co.uk: "We have made strenuous efforts all summer to persuade Jonathan to sign a new long-term contract and in fact held more talks with him as recently as this week.
"As always, Jonathan has been a model professional ever since Fulham first lodged their interest a month ago.
"We have stressed all along that we are not under any financial pressure to sell our best players and that no-one would leave unless it is on our terms. The board and coaching staff firmly believe this is the right deal for the club and we wish Jonathan all the best at Fulham."
