The British number two never looked entirely comfortable with his game against the Spanish clay court specialist, the 2003 French Open champion, and paid for not making the most of opportunities in the first set.
Henman, 32, saved three break points in the fifth game to take a 3-2 lead but after being 30-love up in the next he let slip two break points.
He let another two break points go in the eighth game but, crucially, when he next found himself facing a break against his own serve he volleyed into the net to hand Ferrero a 6-5 lead.
From there the Spaniard closed out the first set as Henman's attempted drop-shot at 40-15 landed outside the line.
Henman suffered another break in the third game of the second set but gave himself an important confidence boost by winning his next service game to love.
But he was still 4-2 down by the time his serve came around again and Ferrero won that one also as he powered his way into the second round at Henman's expense.
