South African Immelman, who had surgery to remove a benign tumour in December, defied 25mph winds to win his first major title by a three-shot margin.
World number one Tiger Woods (72) finished in second place, with fellow Americans Stewart Cink (72) and Brandt Snedeker one shot further back.
Immelman was six clear and cruising with four to play and still five ahead on the 16th tee, but he then carded a double-bogey five.
He hit his approach to the 17th into the guarding bunker but bravely got up and down, and despite unluckily finding a deep divot on the final fairway, Immelman tapped in for par to card a 75 for an eight-under-par aggregate.
Open champion Padraig Harrington (72) finished as the top European in joint fifth place at two under alongside left-handers Phil Mickelson (72) and Steve Flesch (78).
Spain's Miguel Angel Jimenez carded a superb 68 to finish tied for eighth at one under alongside Robert Karlsson (73) and Andres Romero (73), with Nick Watney (71), Lee Westwood (73) and Paul Casey (79) at level par.
