Jacobsen, 28, underwent a medical on Monday and the free agent defender, who was with German club Nurnberg last season, has agreed a one-year deal.
Providing there is no problem with the medical results, Jacobsen will become an Everton player and Moyes will be able to turn his attention to other targets.
Moyes will hope Jacobsen is the first of several signings this week, with the need to bring in new faces now urgent as next Monday's transfer deadline approaches.
The Everton boss accepted at the weekend that his squad needed quantity as well as quality, and suggested he required "six or seven" new players, some of which could be brought to the club on loan.
Former Chelsea midfielder Tiago has already refused a loan move from Juventus, while winger Albert Riera is holding out for a move to Liverpool despite the Toffees meeting the £12million asking price of Espanyol.
Jacobsen will solve a pressing problem in defence for Moyes, with regular right-back Tony Hibbert likely to be out for six more weeks with injury.
Everton have Tim Cahill, Steven Pienaar and Hibbert still some way short of fitness after injuries, although striker Victor Anichebe will be back in the fold this week after winning an Olympic silver medal with Nigeria.
Since last summer, Thomas Gravesen, Andrew Johnson, Stefan Wessels, Manuel Fernandes, Anthony Gardner, Lee Carsley, Jamie Jones and Darren Dennehy have all left the Everton playing staff without being replaced.
Jacobsen may not be a signing to set the pulses racing, but he won three Danish titles with FC Copenhagen and had interested Everton in 2006 before moving to Hamburg.
