With the tourists resuming their innings on 96 for four, Gayle was already playing with a hamstring injury and using a runner, but suffered a blow to his thumb in only the fourth delivery of the day from Andre Nel.
Dwayne Bravo came in, but he lasted only 45 minutes and was out for 12 after flicking a wicked delivery from Nel off his gloves into the grateful hands of South Africa captain Graeme Smith.
West Indies were reeling on 126 for five and they only managed to add seven more runs before Raul Lewis went for one when he nudged the ball to Hashim Amla at short-leg.
Shivnarine Chanderpaul and Jerome Taylor attempted to steady the innings and added 28 runs before the end of the session.
Chanderpaul was unbeaten on 33 and Taylor 21 not out with West Indies holding a 1-0 lead in the three-Test series.
