THIS is the full list of Baby P's contact with the authorities who failed to protect him.

The damning dossier - written by officials and revealed for the first time today - was prepared by prosecution lawyers in the Old Bailey abuse trial and documents the 78 occasions he was seen by social workers, doctors and health visitors.

1 March 2006: Baby P born

22 March: First home visit by health visitor Yvonne Douglas. P has oral thrush

24 March: Family GP Jerome Ikwueke sees P for oral thrush

7 April: Weighed at baby clinic by Yvonne Douglas

13 April: Six-week examination by Dr Ikwueke

2 May: GP visit for diarrhoea and vomiting

4 May: Mother attends health clinic with P, seen by Yvonne Douglas

22 May: First vaccinations for meningitis and diphtheria, pertussis and tetanus at GP clinic

28 May: P vomiting after feeds, mother calls out-of-hours emergency service

4 June: GP visit for pain, diarrhoea and vomiting

9 June: P's mother seen for depression by mental health worker Karolina Jamry

19 June: GP visit, P's second immunisations

11 August: P's mother seen by Ms Jamry about problems in her marriage

15 September: Home visit by Yvonne Douglas

19 September: P seen by GP for nappy rash

13 October: Seen by GP for bruising to head and chest, mother claims accidental fall down stairs

17 November: GP visit for upper respiratory tract infection and thrush

11 December: Admitted to Whittington Hospital. Bruising to forehead and nose, sternum and right shoulder/breast

12 December: P examined and referred to Child Abuse investigation team, seen by Det Con Angela Slade

13 December: P examined on ward by consultant paediatrician Heather Mackinnon

14 December: P examined on ward by Dr Mackinnon

15 December: P discharged into care of Angela Godfrey. Police investigation begins

18 December: Social worker Agnes White visits mother at home

19 December: P's mother and grandmother arrested and interviewed at Hornsey police station

21 December: P's leg X-rayed at hospital

22 December: Mother attends child protection conference with Haringey social workers and Dr Mackinnon

24 December: Emergency duty team visits Angela Godfrey's home to check on P

27 December: Ms White visits P at Godfrey home. Returns later unannounced

29 December: Ms White returns for check on P's contact with mother

9 January 2007: Angela Godfrey takes P to health clinic for thrush on buttocks, seen by Yvonne Douglas

12 January: P's leg X-rayed again at hospital

16 January: Ms White carries out second check on P's contact with mother

17 January: P's leg X-rayed again at hospital

19 January: Mother seen by Ms Jamry

25 January: GP visit for nappy rash

26 January: Repeat visit by Ms White to check on P's contact with mother. Mother seen by Ms Jamry

2 February: P's third set of vaccinations. Maria Ward allocated as social worker

8 February: Preliminary assessment of mother by unspecified official Caroline Sussex

18 February: P and mother moved to new address in Haringey

22 February: Social worker Maria Ward's first home visit

27 February: Miss Ward attends case conference at Haringey

2 March: Miss Ward and health visitor Paulette Thomas see mother at home

5 March: Miss Ward questions mother after school nurse sees her slap a child

6 March: Unannounced visit by Miss Ward

8 March: Visit by Miss Ward

14 March: Visit by Family Support Service worker Marie Lockhart

16 March: Haringey Child Protection conference with mother, attended by Miss Ward and Miss Lockhart

20 March: Mother and P videoed at parenting class

22 March: Miss Ward visits

23 March: One-year check at health clinic

29 March: Haringey case conference

9 April: P seen by GP with bruising to face. Mother claims he was pushed into fireplace by another child

9 April: P admitted to North Middlesex hospital for bruising and swelling to head

10 April: P referred to child development clinic by social worker who sees him "headbanging"

11 April: Discharged from N Middlesex

12 April: Child Protection meeting at North Middlesex hospital

24 April: Miss Ward home visit

3 May: Mother and P attend parenting class

9 May: Planned home visit by health visitor Ms Thomas

16 May: Family support visit by Ms Lockhart

18 May: GP visit for hives (allergic reaction)

21 May: Miss Ward visits

1 June: Miss Ward unannounced visit, reports mother to police over bruises to P. P taken to North Middlesex for check-up 5 June: Mother interviewed under caution at Hornsey police station

6 June: P seen by Ms Thomas at health clinic

7 June: More immunisations at GP surgery and child protection meeting at North Middlesex

8 June: Police take photos of P and seize toy from home

12 June: Registered childminder Anne Walker takes P for day care for 10 days

15 June: Ms Lockhart visits home

19 June: Miss Ward visits childminder

20 June: Case conference at Haringey

21 June: P and mother attend parenting class

5 July: Mother and P attend parenting class

9 July: Mother takes P to North Middlesex with ear infection

11 July: Miss Ward visits

18 July: P seen at health clinic for scalp and ear infection

19 July: Mother and P attend parenting class and attend North Middlesex with previous infection

26 July: GP visit for head lice and blood in ear

30 July: Case conference at Haringey, Miss Ward home visit, mother feeling stressed

1 August: P seen at St Ann's Hospital by locum paediatrician Sabah Al Zayyat

2 August: Mother told police would take no further action over assault allegations

3 August:

11.36am, 999 call.

11.40 Ambulance arrives.

11.43 Ambulance leaves.

11.49 Ambulance arrives hospital.

12.10pm P pronounced dead. Police called.

13.30 Body maps completed.

13.45 Mother arrested.

I am very concerned about a situation that is developing in England about a family who is receiving no help or advice from NHS - rather the opposite. please if anyone can help me by forwarding any information of helplines other than NHS please do so immediately.time is running out.

where is the justice? whos to be blamed for this tradgic death? why are all parties involed opting out insisting they have done thier job well? I want to see BABY PETERS mother, boyfiend and the lodger face life in prision. I want BABY PETERS mother to have her womb taken out as goes the same for the boyfriend and lodger.
I want a change to happen. These are our children dying at the hands of powers that were established to protect them. Who will admit they did wrong by BABY PETER?

he was a beatiful baby and no person desevres that much torture.People like his mum and step dad dont desreve to have children. and social servies need to sort out there way of working and prosejers.This case has been going on for about a year and if someone cared that much they would have took the baby of them

Aw Baby.P is soo cute why wud his parents b soo cruel to him he is soo sweet if they didnt want him anymore and were getting fed up they cud of gave it to social services to look after him instead of torturing him breaking his soft little ribs. I felt like crying over his death and thinking how he must of felt at that tym I Love You Baby.P R.I.P

It is absolutely heartbreaking to imagine the pain and suffering this beautiful child of God must have endured at the hands of these animals.
Imagine if the mother had instead, at birth, relinquished this beautiful boy to an adoptive family who would have loved and cared for him like the angel that he was. Who knows what great things he could have achieved in his life. I do not pity the three monsters who WILL NEVER AGAIN enjoy one single, peaceful day for the rest of their lives, for this incident will rest in their hearts and souls for ever.

One of the most chilling things to come from this list is the fact that 'the school nurse' reported the mother, and later the mother suggested Baby P was pushed into a fire place by 'another child' - Baby P clearly has a sibling: what has he or she also suffered?

This case is absolutely heartbreaking. I cannot get that poor little boy out of my head. What that poor child must have gone through is beyond belief. I am not sleeping properly after hearing and reading about the details of what happened to him and when I see his innocent little face it literally causes me to feel sick and fill up with tears. I keep getting hurrendous thoughts of the poor little thing knowing nothing but suffering, fear and pain in his very brief life. It's haunting me. How can another human being treat such a beautiful, harmless, innocent child in this way?Something really needs to be done to protect the vunerable children in our society. My heart aches for that little boy it really does. He's been so let down.

DREADFUL.....totally shockin, I hope they all had a nice christmas, because it horrible 2 think what P's Christmas was like. the poor little lad deserves to have justice done for what 'THE SYSTEM' did to him,.........
(yes, i know the sic people who actally inflicted injury to P arent in the system, but if I write what I feel about them, or what shold happen to them, my post would just be removed !!)

This is a tragedy, why was nothin done? All the times this child was seen by hospital, childminders and everyone else and they saying nothin seemed parculiar. Its a load of rubbish. This baby should of and could of been saved but instead got to stay living with the creatures that were meant to be taking care of him being his PARENTS. They should be tortured now till the day they die, hope the rot in hell. R.I.P Baby P xxx God bless you!
