A paedophile was able to rape a five-year-old girl he was babysitting because agencies failed to tell her mother he was a convicted sex offender.
A serious case review revealed a number of agencies failed to provide adequate protection for the child.
At the time of the attack, the man was receiving treatment and was being supervised by Cleveland Police and the National Probation Service Teesside and Durham.
Jane Humphreys, chairman of the Stockton-on-Tees Local Safeguarding Children Board, said: "Speaking on behalf of all the agencies, we are profoundly sorry for our failures in this case. We particularly regret we did not provide the mother of this child with information to enable her to adequately protect her daughter. We are continuing to offer support to the family.
"The well-being of children is of utmost importance and we deeply regret the impact this case had on the child. We fully support the report's recommendations and will continue to work in partnership across all agencies to address the shortcomings highlighted to strengthen the services we offer."
The man, from Billingham, Teesside, was jailed for six years at Teesside Crown Court last year after admitting five counts of rape.
He was also ordered to register as a sex offender for life and banned from recreation centres and from having contact with children.
While in contact with the young girl, he was undergoing sex offender treatment as part of a two-year supervision order imposed after he admitted child pornography offences.
He was being jointly supervised by Cleveland Police and the National Probation Service.

It just goes on and on, always learning but never implementing the lessons.
Poor child. The human rights act is a charter for Criminials written by criminals so that they can escape the consequences of their actions.
When will common sense prevail and the Law of Yahweh be re-established?
Old aged people are prosecuted for not paying taxes they can no longer afford but criminals walk free.

This is yet another truly appalling......horrifing case of digraceful failure of the agencies involved to do their job and what do we get.............

How about ensuring this man could not work with, or anywhere near, children ? Why has the order banning him from recreation centres and having contact with children only just been handed down?
Those who have singularly failed to do their job, trot out the usual platitudes which just sound like "Damage Limitation" statements brimming over with self-protection and little regret, except at the adverse publicity. Why is there no accountability by ANYONE ????
How could anyone with this man's record be able to work as a baby sitter???? or in ANY capacity whatsoever that gives him contact with children????

This is an absolute disgrace and NO amount of apologising will ever make this right. How can they possibly have failed to inform any parent that this man was on the sex offenders register for looking at child pornography. It simply beggers belief.

This is so infuriating and depressing. Will anything ever be learnt from this? Just about every member of the public is amazed and disgusted by this type of thing happening again and again, yet nothing changes. It's unbelievable that a known paedophile who was being supervised by the police and the National Probation Service was allowed to work as a babysitter. It's been shown time and time again that they cannot properly supervise these people, so either keep them locked up or let everyone else know who they are so they can supervise them jointly.
